About Amberfield

About the Role
The Dementia Manager provides clinical leadership and operational oversight, creating a safe, supportive and truly home-like environment where people living with dementia can continue to live well. This role is responsible for leading the delivery of high-quality, person-centred care and support that reflects LDK’s Dementia Vision - enabling residents to experience connection, purpose and belonging - while promoting dignity, choice, independence and each person’s unique journey.
Care is supported by a consistent team of Registered Nurses and Personal Care Assistants providing 24/7 support in our two purpose-built Memory Support cottages - Rosemary and Rowley - each supporting a small community of around 10 residents.
These cottages are designed to deliver truly individualised, relationship-based care in a calm, home-like environment, complemented by secure cottage gardens that support safe outdoor access, sensory engagement, and meaningful connection to nature.
Role Responsibilities
- Provide clinical and operational leadership of the cottage’s care team
- Deliver safe, high-quality, evidence-based, person-centred care
- Foster a safe, supportive and dementia-friendly environment and promote meaningful engagement and quality of life for residents
- Build trusted relationships with residents and families, and monitor well-being
- Work in partnership with a dedicated Care Partner supporting the cottage
- Build team capability through recruitment, coaching and development
- Coordinate with rostering to ensure safe and effective staffing
- Liaise with GPs, Nurse Practitioners and multidisciplinary teams
- Lead incident management, risk mitigation and continuous improvement
- Participate in an on-call roster for clinical escalations (as required)

About you
- Registered Nurse with current AHPRA registration
- Minimum 5 years’ clinical experience, ideally in aged care
- Strong experience supporting people living with dementia
- Confidence managing behaviours and complex care needs
- Skilled in person-centred care and behaviour support strategies
- Understanding of psychotropic medications and restrictive practices
- Proven leadership experience supporting and developing teams
- Enjoy building strong positive connections with residents, families and team members.
- Community or Home Care experience (desirable)
- Additional dementia training or postgraduate study (desirable)
