About the Role
The Practitioner, Sustaining Tenancies role is focused on providing person-centred early intervention and prevention support to individuals and families who are at risk of homelessness. In this role, you will work closely with clients to help them maintain stable housing by delivering ongoing assessment, tenancy support, and post-crisis assistance tailored to their changing needs. The position involves empowering clients to build independence, strengthen their self-reliance, and connect with appropriate community and support services. Practitioners are expected to provide compassionate, strengths-based support while working in line with the values and mission of Anglicare Southern Queensland, contributing to positive outcomes for vulnerable individuals and families within the community.
This is a permanent Part time position, 30.4 hrs working 4 days per week, based in Toowoomba , QLD.
Responsibilities
- Provide person-centred case management and ongoing support to individuals and families at risk of homelessness.
- Develop trauma-informed case plans, facilitate goal setting, and implement support strategies tailored to client needs.
- Assist clients to obtain and sustain safe housing while promoting independence, self-reliance, and connection to community support.
- Monitor housing stability, conduct risk assessments, and implement strategies to minimise risks and improve client outcomes.
- Maintain accurate client records, complete administration tasks, and meet data collection and reporting requirements.
- Deliver professional, ethical, and strengths-based support in line with organisational policies, procedures, and quality standards.
- Participate in integrated service delivery, local housing networks, team meetings, and continuous improvement initiatives.
- Contribute to a positive team culture through collaboration, peer support, professional development, and ongoing training.
Qualifications, Credentials and Other Requirements
- Degree in Health, Human Services, Behavioural Science, Social Work, Psychology or similar, and/or equivalent practical experience.
- Current Queensland Driver Licence
- Current Blue Card (Working with Children Check)
- Current National Police Check
- Experience with intervention strategies including tenancy sustainment, homelessness, family support, at risk individuals
- Strong written and verbal communication, negotiation and relationship building skills.
- Ability to work both independently and collaboratively in a team environment.
- Knowledge and skills in housing/mental health/alcohol and other drugs/domestic and family violence
- Commitment to flexible working hours as required.
