A temporary opportunity is available within SESLHD to coordinate the Out of Hospital Care program. Use your coordination skills and clinical judgement to apply a strategic and flexible approach to managing this program.

The Out of Hospital Care Coordinator will work in close collaboration with program providers and internal and external referrers to ensure the Out of Hospital Care (OHC) Program (Community Packages -ComPacks, Safe and Supported at Home (SASH) and End of Life Care packages) across SESLHD and St Vincent’s Health Network achieves its business and service objectives.

Selection Criteria
- Relevant tertiary qualifications in a health discipline or relevant equivalent work experience related to aged care, disability support and palliative care in a variety of health settings, or a combination of study and work experience.
- Extensive knowledge of contemporary issues in aged care, disability support and palliative care.
- Demonstrated high level verbal and written communication skills, interpersonal, negotiation and influencing skills with a strong customer service approach.
- Demonstrated high level analytical and problem solving skills including the ability to provide authoritative advice and recommendations across a large and complex organisation.
- Demonstrated ability to develop and maintain effective working relationships with senior management, and other key internal and external stakeholders.
- Demonstrated skills in organising and prioritising competing and shifting demands in a high-volume dynamic work environment and managing competing projects.
- Demonstrated experience in program planning, implementation and evaluation at strategic and service levels.
- Willingness to travel in accordance with the demands of the position.
