We are seeking a motivated and experienced Registered Nurse – Diabetes Education to join our team and deliver high-quality, patient-centred diabetes care. This role focuses on supporting individuals to effectively self-manage their diabetes through education, clinical support, and evidence-based practice across a range of care settings. You will work collaboratively with multidisciplinary teams to improve health outcomes and enhance the quality of life for people living with diabetes. Where you’ll be working : Hornsby Ku-ring-gai Hospital, Diabetes Education Team.
What you’ll be doing: Provide clinical care and education to patients across the diabetes continuum, Deliver structured and individualised diabetes self-management education, Support patients with insulin initiation, medication management, and glucose monitoring, Promote prevention and early intervention strategies for diabetes-related complications, Collaborate with multidisciplinary teams to coordinate holistic care, Develop and deliver education programs for patients, families, and staff, Participate in quality improvement initiatives, audits, and service evaluation, Maintain accurate documentation and contribute to clinical reporting requirements and Support culturally appropriate and patient-centred care delivery.

Candidates Will Need To Meet The Following Criteria
- Registered Nurse - Division 1 with Australian Health Practitioner Regulation Agency (AHPRA).
- Demonstrated effective interpersonal, verbal and written communication skills.
- Current clinical knowledge and clinical problem solving ability.
- A commitment to excellence in nursing practice and continuing professional development.
- Ability to work effectively within a multi-disciplinary team.
- An understanding of the professional, ethical and legal requirements of the Registered Nurse.
