We are seeking a Care Finder to join our Aged Care team. In this role, you will:

  • Provide specialist, intensive support to help individuals understand and access aged care services, while connecting them with relevant community supports.
  • Focus on supporting people in the Mitchell and Whittlesea shires who require additional assistance to engage with My Aged Care and navigate available services.
  • Deliver proactive outreach, building strong engagement and rapport with potential clients.
  • Guide clients through the assessment process and support them in accessing the aged care services and supports they need.

You are a high‑performing aged care professional who brings sound judgement, strong organisational skills, and a client‑centred approach.

You will have:

  • Tertiary qualification in Human Services/Social Work or Community Services and or related profession and or previous experience in needs assessment, care planning and/or case management
  • Experience in aged care aged care, health and community services system and regulatory environment
  • Experience in working with older people and their families/carers from a range of backgrounds, and an understanding of the barriers they may experience in accessing aged care services
  • Excellent communication and stakeholder engagement capability
  • Proven ability to manage competing priorities
  • Computer literacy and the ability to effectively manage a client data base
  • High level problem solving, exceptional customer service skills and analytical skills
  • Excellent IT skills, including Microsoft Office suite

The fine print

  • Successful candidates will be required to undergo a National Police Check, International Police Check (if applicable), hold a valid WWCC and a current Victorian Drivers License.
