About the Role
We’re on the lookout for a passionate Care Manager to join our Toowong team and make a real impact in the lives of older Australians.
In this role, you’ll lead the entire client journey — from onboarding and personalised care planning to coordination and ongoing reviews. You’ll be the key connection point between clients, families, caregivers, and health professionals, ensuring our clients feel supported, heard, and empowered every step of the way.
You’ll be based in Toowong with travel across Brisbane Northern, Brisbane Central, Brisbane Southern and Ipswich regions — so every day brings variety and meaningful connection.

What You’ll Do
- Design and manage individualised care plans that put the client first
- Build warm, trust-based relationships with clients and families
- Coordinate services and ensure the highest quality of care
- Work collaboratively with caregivers and health professionals
- Keep accurate records and share updates with the wider team
About You
- Experience in aged care or case management (HCP experience a bonus!)
- Great admin, time management, and communication skills
- Tech-savvy with Outlook, Word & Excel
- Cert III in Aged Care (desirable, but not essential)
- A valid driver’s licence + willingness to travel locally
