What you'll be doing

In this dynamic role, you will deliver comprehensive, recovery-focused mental health care to older people living in the community, including those with complex needs such as dementia and behavioural challenges.

• Conduct comprehensive assessments and develop collaborative care plans

• Deliver evidence-based interventions, care coordination and ongoing support

• Work closely with multidisciplinary teams and external service providers

• Support consumers, families and carers through education and therapeutic interventions

• Manage risk, respond to crises and ensure safe, timely care delivery

Essential Requirements

• Qualification: Registered Nurse, Occupational Therapist, Psychologist or Social Worker

• Registration: Full registration with AHPRA as Registered Nurse, Occupational Therapist or Psychologist or Social Worker eligible for membership of AASW

Selection Criteria

• Significant clinical experience in the field of aged care and/or mental health.

• Experience and ability to conduct comprehensive mental health assessments, including risk assessments, and to develop and implement care plans and interventions in collaboration with consumers and carers.

• Demonstrated excellent written and verbal communication skills

• Highly-developed interpersonal skills with an ability to effectively develop a therapeutic relationship with consumers and carers from diverse backgrounds.

• Demonstrated ability to work effectively both as a member of a multidisciplinary team and independently.

• An understanding of recovery models in mental health and the ability to implement these within clinical practice.

• An understanding of the NSW Mental Health Act (2007) and the Guardianship Act (1987).

• Current Drivers Licence valid within Australia.
