A locally owned and operated Registered NDIS provider that offers Aged Care & Disability Care across Brisbane is looking for an experienced Registered Nurse to join their team as their Care Coordinator.

About the Role and your Responsibilities

Reporting to the Managing Director, you will play a pivotal role in safeguarding clients' well-being and coordinating clients’ care needs, including assessment, case management, and development of an individualised Care Plan. Employing your professional skills and knowledge, you will assist clients in developing attainable goals and, in turn, coordinate a range of services that support and promote these goals.

Ideally, you will be someone who can maintain a high level of professionalism and work as a dependable member of the team. Some of your responsibilities will include:

  • Client assessment, developing tailored care plans, reviews and case management
  • Managing budgets and coordinating care services to enhance the client’s quality of life and maintain their independence
  • Clinical in-home services including, medication, incontinence, wound management and diabetes management.
  • Coordinate the delivery of all services by Support Staff, including scheduling/rostering
  • Ensure the HCP and/or NDIS programs runs effectively and efficiently on a day-to-day basis
  • Work Health and Safety issues
  • Risk Management
  • Maintain the administration practices and procedures inline with the organisations standards
  • On Call - Shared across the team on a rotational roster
  • Provide prompt and solution-oriented service to clients
  • Address and manage client complaints/feedback

Skills and Qualifications

  • AHPRA Registered Nurse with current registration status
  • 2 years prior experience in Aged Care or Support Coordinator for Home Care Packages Program/ Support At Home Program and/or NDIS service provider
  • Certificate IV in Assessment & Training - Desirable
  • Experience in coordinating programs for older people or people with a disability
  • Knowledge of the Aged Care Quality Standards and/or NDIS Quality Standards
  • A consumer directed care focus
  • Excellent documentation and computer skills
  • Ability to liaise and consult with a wide range of clients, health professionals and team members
  • Excellent communication and interpersonal skills
  • Own registered vehicle and current driver license
  • NDISWC Clearance and Police Check
  • Right to work in Australia
