POSITION DETAILS
The Team Leader Care Finder coordinates and supports delivery of the Care Finder program across the region, ensuring high quality person centred services aligned with program requirements. The role provides day to day guidance practice support and oversight to Care Finders to ensure consistent and effective service delivery to older people facing barriers to accessing aged care and community supports.
RESPONSIBILITIES
Program Management
- Manage the Care Finder program to ensure compliance with funding guidelines contracts legislation and reporting requirements
- Support implementation of aged care reforms and ensure team understanding of program expectations and service delivery changes
- Oversee complaints feedback and incidents including investigation resolution and escalation in line with policy and regulatory requirements
- Build and maintain strong stakeholder partnerships to support referrals program outcomes and community engagement
- Ensure accurate data collection documentation reporting and program evaluation aligned with quality standards and contractual obligations
- Drive continuous improvement including development of policies procedures and program practices in consultation with the Regional Manager
People Management
- Provide leadership coaching and support to Care Finder staff to ensure consistent high quality service delivery
- Promote best practice person centred care focused on dignity choice wellbeing and independence
- Build team capability through supervision performance reviews and case conferencing
- Support recruitment workforce planning and staff development in collaboration with management and HR
- Monitor staff performance caseload management and wellbeing to ensure effective operations
- Maintain regular communication and reporting with the Regional Manager to support program outcomes
KEY SELECTION CRITERIA
Essential
- Relevant tertiary qualification in social work social science human services or related discipline or significant experience in a leadership role within community services
- Strong understanding of person centred service delivery including client led assessment case management and support planning
- Knowledge of aged care legislation and current reforms with the ability to translate requirements into effective service delivery
- Demonstrated leadership capability including team development performance management and collaborative practice
- Experience working with diverse client groups including older people people with mental illness disability and those experiencing or at risk of homelessness
- Strong stakeholder engagement communication and organisational skills with the ability to manage data reporting budgets travel requirements and use digital systems
- Must have a current driver licence, access to a comprehensively insured vehicle and willing to obtain a NDIS Workers Screening Clearance
Desirable
- Travel throughout Care Finder delivery regions and stay overnight were needed for the length of the program.
- Current First Aid and CPR Certificate.
