Senior Complex Needs Coordinator

THE ROLE

The Complex Needs Coordinator provides consultation and coordination services for clients with complex support needs and is a single point of contact for service providers, clinicians or people seeking general information about complex needs service responses/interventions. Complex needs service responses/interventions include Multiple and Complex Needs Initiative (MACNI), the Support for High Risk Tenancies (SfHRT) program and broader complex needs responses.

The position will:

  • Provide consultation and coordination services and facilitate service planning for clients with complex support needs, including MACNI and SfHRT clients
  • facilitate relationships between departmental program areas and external services, as well as provide capacity building regarding support provision for clients with complex needs
  • partner with service providers to develop integrated responses and promote practice excellence
  • identify systems gaps, risk factors and training opportunities that will improve service systems for clients with complex needs
  • support the Area Client Support and Housing unit and represent the program in various environments.

ACCOUNTABILITIES INCLUDE

  • Provide or contribute to authoritative and expert advice and recommendations with a specific program or area-based focus, consistent with organisational goals and
  • In collaboration with the relevant Manager, manage the allocation of resources within and the outputs required of the work area to achieve service delivery goals and
  • Interpret and apply business plans and policies in own area of responsibility and provide advice to others on design and implementation
