Ozcare has a full-time Care Partner position available at our Community Care branch based in Hervey Bay. We are looking for a compassionate individual who can help support our older generation in living independently.

Your Role

You'll be responsible for planning, managing and leading client care and support across a range of programs. You'll also play a key role in coordinating the involvement of Ozcare's multi-disciplinary teams and other services to support effective care.

As a Care Partner you will:

  • Manage and coordinate care services to ensure the highest standards of care
  • Develop and implement care plans tailored to individual client needs
  • Liaise with clients, families, and healthcare professionals to ensure comprehensive care
  • Foster strong relationships with clients, stakeholders, and team members, while assisting clients in meeting their unique needs and goals
  • Ensure compliance with all relevant legislation and organizational policies
  • Monitor and evaluate care services to continuously improve quality and efficiency
  • Monitor the budget of Home Care Packages and ensure clients understand their funding and fees

For Success you need to

  • Demonstrate experience in case management and client care coordination in a Community Aged Care setting
  • Have demonstrated strong verbal, written and interpersonal communication skills to communicate and work effectively with a broad range of people
  • Be vaccinated against Influenza & at least 1 dose of Covid-19 vaccine
  • Provide proof of immunity, or willingness to become vaccinated against, Measles, Mumps, Rubella (MMR), Varicella (Chicken Pox) & Pertussis (Whooping Cough)
  • Hold a current Police Screening Certificate
  • Provide a copy of your current First Aid and CPR Certificate
  • Hold a current NDIS Worker Screening Check (Yellow Card)
  • Hold a current Queensland driver's licence
  • Completed tertiary qualifications and/or relevant experience in a health-related field (e.g. Health and Human services)
