This fixed-term role has been created to provide strategic and operational leadership to the Home Care Services and Community Health Nursing teams during the secondment of the current Program Manager. The position is central to ensuring service continuity, building team capability, and identifying opportunities to enhance client experience across both the Home Care and Community Nursing portfolios.
Key areas of responsibility
- Program Leadership — Provide leadership to the Post Acute Care, Community Health Nursing, and Support @Home teams. Working closely with Team Leaders, oversee day-to-day operations, ensure service targets are achieved, support staff performance and wellbeing, and uphold strong clinical and operational governance. Lead planning and initiatives that contribute to the rollout of BHN’s new strategic plan, with a particular focus on growth within the Support at Home program.
- Service Optimisation — Identify and implement opportunities to streamline processes, improve client flow, strengthen communication pathways, and enhance integration across teams. This includes analysing current practices, recommending improvements, and supporting change initiatives that align with BHN’s strategic direction.
