Role Description
This is a full-time, on-site role located in Hobart, TAS, for a Specialised Support Coordinator at Apoyo Health Services. The coordinator will provide personalized case management services, act as a point of contact for clients, design and implement support plans, and collaborate with service providers and community resources to meet client needs. Additional responsibilities include ensuring compliance with NDIS guidelines and offering exceptional customer service in a supportive environment.
Qualifications
- Strong Interpersonal Skills and the ability to build and maintain positive relationships with clients and stakeholders
- Effective Communication and active listening skills to understand and address client needs
- Experience in Case Management, including planning, coordination, and monitoring support services
- Customer Service skills with a commitment to delivering a client-first approach
- Technical Support capabilities for navigating NDIS systems and documentation processes
- Attention to detail, organizational skills, and problem-solving abilities
- Familiarity with NDIS frameworks and policies is advantageous
- Certificate IV or higher in Community Services, Social Work, or a related field is preferred
