We’re looking for a highly organised, relationship-driven professional to play a pivotal role in the rollout of the new Support at Home (S@H) model supporting older adults to receive the care they need, their way.

Why this role matters

This is more than coordination—it’s about creating seamless client experiences during a major national aged care transition. You’ll be the vital link between clients, clinicians, and providers, ensuring services are connected, timely, and high quality.

What You’ll Be Doing

  • Lead the transition of clients from CHSP to Support at Home services
  • Act as the key liaison between BHN, clients, and external providers
  • Manage intake, referrals, and support plans with precision and care
  • Coordinate provider agreements and resolve invoicing queries
  • Support clinicians with reporting and ensure data accuracy for billing
  • Monitor and track support plan renewals to keep care uninterrupted

What You Bring

  • Experience in aged care, community services, or health administration
  • Strong coordination, intake, or provider engagement experience
  • Exceptional attention to detail and confidence working with data systems
  • Ability to manage competing priorities and multiple stakeholders
  • Advanced skills in Microsoft Office (especially Excel)
  • A proactive, solutions-focused mindset

Bonus points for experience with TrakCare or aged care funding models (CHSP, HCP, S@H).
